Methodist Le Bonheur Healthcare's CDSMP is state funded program designed to provide people with chronic diseases, and/or their caretakers, with essential tools needed to manage their health condition.

Participants will attend a 2.5 hour health education class every Monday for six weeks. During classes, participants will receive information that can help better manage their chronic disease and its symptoms.

Program Qualifications

To qualify for this free program, you must identify as having a chronic disease or being the caretaker of an individual with a chronic disease. Chronic diseases may include:

  • Cardiovascular disease
  • Cancer
  • Asthma
  • Diabetes
  • Arthritis
  • Lung disease
  • Other chronic diseases
